Patients in respiratory distress can benefit from being in the prone position (lying on their stomachs), whether they are on a ventilator or not. Prone positioning (proning) seems to be especially helpful to COVID patients.
In ARDS, proning makes the ventilation pressure more even across the front and back of the lungs and can open up collapsed portions of the lung. Patients can and do self-prone; see below for a video to share with patients. However, proning must be done carefully as it can also be a way to spread infection. In addition, some conditions preclude using proning, like pregnancy or persistent mean aterial pressure <65 mmHg. Proning can also lead to problems like dislodging tubes and catheters, pressure sores, or nerve injuries. Many ideas have appeared online of various degrees of reliability, usefulness, and practicality. Our team is looking at those online resources and has found a few that we feel are the most reliable and practical. Other online resources we found are also listed below.
Proning seems to be uniquely helpful for COVID patients, but is not appropriate for all and should be done carefully to avoid spreading infection.
